How to Sign Athletic Participation Forms Electronically

 

The Granger Athletic Department is continuing to have parents sign athletic participation forms electronically for the 2022-2023 School year.  DragonFly Max is an electronic health record to save time and ensure our athletes are healthy and ready to participate in athletic competition.  DragonFly MAX will allow our coaches and parents to easily communicate with our Baylor, Scott and White athletic trainer.

THE FOLLOWING FORMS ARE REQUIRED. Seven will be completed on the DragonFly Application/Two will be completed via Paper Copy.

Athletic Forms Required to athletic participation at Granger I.S.D.

  • 1. Acknowledgement of Rules
  • 2. Authorization and Consent to Treat
  • 3. Emergency Treatment Authorization
  • 4. Lion Oath
  • 5. Steroid Agreement
  • 6. Sudden Cardiac Awareness
  • 7. Electronic Signature Consent
  • 8. Pre-Participation – Medical History (must have Medical History on file in PAPER FORM)
  • 9. Pre-Participation – Physical Examination (must have physical on file in PAPER FORM)

 

If you already have a DragonFly Max Account for your child from previous years, simply log on to your account and click “Get Started” for the 2022-2023 School year. Be sure that you are under the correct school year: 2022-2023.-IF NOT SHOWING ON THE APPLICATION from your phone, be sure to log on to a computer to complete the paperwork. Mobile application for paperwork will not be available until June, however, computer-based paperwork is available now.

New to DragonFly? Please follow these easy steps to get started using DragonFly MAX.

 

“I’M A PARENT”:

  1. Visit http://dragonflymax.com/forms/F4RXWQ.  This will take you to the Granger Independent School District DragonFly MAX sign up page, and click “GET STARTED”.
  2. On the sign up page, click “Sign Up for Free”.
  3. Follow the prompts to create your Parent Account with your email address or phone number.  You will receive an email or text asking you to verify your account information.
  4. Once you are able to sign into DragonFly MAX, click “Add A Child” in the DragonFly MAX web site, then follow the prompts to create your child’s profile and complete his/her participation forms.  All Granger I.S.D. Athletic Participation forms can now be signed electronically except for the Medical History and Physical Exam.  The Medical History must be filled out and signed by the parent and athlete, and the Physical Exam (if needed) must be filled out and signed by a physician.
  5. After completing your child’s forms, you can review his/her profile OR add another child’s profile.

 

Now that you’re done, download DragonFly MAX from the App Store or Google Play and sign in.

“I’M AN ATHLETE OR COACH”:

  1. Visit http://dragonflymax.com/forms/F4RXWQ.  This will take you to the Granger Independent School District DragonFly MAX sign up page, and click “GET STARTED”.
  2. On the sign up page, click “Sign Up for Free”.
  3. Follow the prompts to create your Athlete or Coach account with your email address or phone number.  You will receive an email or text asking you to verify your account information.
  4. Once you have verified your account, you can sign in to DragonFly MAX and enter all of your required information.

 

Use the QR code at the top of the page to go straight to the Granger Forms Sign Up on DragonFly Max.

Now that you are done, download DragonFly MAX from the App Store or Google Play and sign in. APPLICATION USE WILL NOT BE AVAILABLE UNTIL JUNE 1st, 2022, for the 2022-23 school year. You must complete the forms on the desktop site if completing forms before June 1st, 2022.
